Members unreachable due to the nature of employment and shifts worked.
Interventions are subjective including disease stratification and management thereof.
Negative impact on member early diagnosis, access to treatment, follow up and compliance management.

The core services will include and are not limited to:

Individual house calls including counselling, and follow-ups,
Treatment planning, medication monitoring and management,
Assessment of compliance,
Assistance with access to community resources, supportive counselling for specific needs, and family involvement as and when the need arises.

High blood pressure and other diseases of the heart and blood vessels.

Diabetes mellitus and a few other metabolic disorders.

Chronic respiratory diseases like asthma.
