AMBULATORY DISEASE RISK PROGRAM
Office driven traditional approach of chronic disease management has been proven not the successful way to chronic disease management.
- Members unreachable due to the nature of employment and shifts worked.
- Interventions are subjective including disease stratification and management thereof.
- Negative impact on member early diagnosis, access to treatment, follow up and compliance management.
The core services will include and are not limited to:
- Individual house calls including counselling, and follow-ups,
- Treatment planning, medication monitoring and management,
- Assessment of compliance,
- Assistance with access to community resources, supportive counselling for specific needs, and family involvement as and when the need arises.
