AMBULATORY DISEASE RISK PROGRAM

Office driven traditional approach of chronic disease management has been proven not the successful way to chronic disease management.

  • Members unreachable due to the nature of employment and shifts worked.
  • Interventions are subjective including disease stratification and management thereof.
  • Negative impact on member early diagnosis, access to treatment, follow up and compliance management.

The core services will include and are not limited to:

  • Individual house calls including counselling, and follow-ups,
  • Treatment planning, medication monitoring and management,
  • Assessment of compliance,
  • Assistance with access to community resources, supportive counselling for specific needs, and family involvement as and when the need arises.

Management of Non-communicable disease (NCDs)

  • High blood pressure and other diseases of the heart and blood vessels.
  • Diabetes mellitus and a few other metabolic disorders.
  • Chronic respiratory diseases like asthma.
